How do I change the name/address on my personal licence?

Please note, before applying to amend your personal licence online, you need to ensure you are changing the information with the Local Authority who originally issued your personal licence.

If you are a Personal Licence holder and have had a change of name or address, you will need to complete the application form below.

You are required to return your current licence (both parts - badge and paper licence), if not provide the reason why and pay therelevant fee.

When completing our online form a card payment is required. We accept most major debit cards and credit cards.

If you are informing us of a change of name you will need to supply proof of change of name (for example marriage certificate/deed poll certificate)
