How do I surrender a Premises Licence or Club Premises Certificate?

If you would like to surrender your Premises Licence you are required to complete and return the Surrender Premises Licence application form (PDF) [470KB] .  You should also enclose both parts of the original Premises Licence.

If you would like to surrender your Club Premises Certificate you are required to complete and return the  Surrender Club Premises application form (PDF) [467KB] (opens new window) .  You should also enclose both parts of the original Club Premises Certificate.
