Environmental Health Record Sheets
In order to decide whether we can take further action over your complaint we require more information.
We need to know how often it is occurring, at what time, how long it is happening and also how the problem affects you.
To do this we require you to complete written record sheets.You can download them from this page. We have provided them as a word document or a PDF.
If you download the word document you will be able to type the information into the document using your keypad.
If you download the PDF you will have to print it off and complete this by writing on it.
You will have received an email with a link to your complaint case and you will be asked to upload your sheets once you have collated 14 days worth of records.
We require a minimum of 14 days of records in order to be able to investigate your complaint.

Below is an example of how we would expect the record sheet to be completed. Please ensure that the add your case number to the sheets in the top left hand corner where it says SRU number. It will look similar to this 6274-3888-8912-4432 and will be on the email we have sent you regarding your case.
If you are unable to upload your record sheet you are able to post it to us or bring it into our office in Dereham.
Please ensure though that you reply to the message on your case that asks you to provide the record sheets and let us know that you are either posting them or bringing them into the office.
